Phimosis

Phimosis is a term used to describe difficulty retracting the penile prepuce covering the head (glans) of the penis. Phimosis may appear as a tight ring or “rubber band” of foreskin around the tip of the penis, preventing full retraction. There are both physiologic and pathologic forms of phimosis, which can complicate the use of the term.

Physiologic phimosis: Children are born with tight foreskin at birth and separation occurs naturally over time. It is common in children between 2 and 4 years of age, is a self-limiting, and resolves once the foreskin becomes more retractile.

Pathologic phimosis: Phimosis that occurs due to scarring, infection or inflammation. Forceful foreskin retraction can lead to bleeding, scarring, and psychological trauma for the child and parent. If there is ballooning of the foreskin during urination, difficulty with urination, or infection, then treatment may be warranted.

Treatment for Phimosis – Circumcision

Grading of Phimosis

Meuli et al. have graded severity of phimosis into following 4 grades, namely,

Grade I— fully retractable prepuce with stenotic ring in the shaft,
Grade II—partial retractability with partial exposure of the glans,
Grade III—partial retractability with exposure of the meatus only, and
Grade IV—no retractability

Treatment of Phimosis

Circumcision is the surgical treatment for Phimosis. Following are the different modalities of doing it:

  • Open Cut the Foreskin & suture

  • Laser Cut the Foreskin & suture 

  • Stapler Circumcision

  • Dorsal slit in case of emergency or unfit patient
